Once again this year, health insurers are doing their best to convince you to choose them.
But… they’re not telling you the full story.
As a result, you may end up paying more than you expect.
Unfortunately, we’re seeing it again:

Health insurance premiums are increasing

Coverage is decreasing
-
Reimbursement for non-contracted physiotherapy is being reduced even further
In 2026, you may receive only a small reimbursement for your treatments — and this is not because of our prices.
So what’s really going on?
The big picture
As a customer, you see dozens of different health insurers. But did you know:

There are actually only seven major insurance groups?

And that 80% of insurers fall under just four parent companies: Achmea, CZ, VGZ and Menzis — also known as the “Big Four”?

Because the basic health insurance package is regulated by law, insurers try to differentiate themselves through their supplementary packages to stay competitive.
They keep the prices of these packages low — and that has a major impact on the physiotherapy sector in particular.

Insurers try to sell more supplementary packages

At the same time, they pay less for the actual care

Contracted therapists receive (too) low compensation

Non-contracted therapists — like us (and therefore you) — receive even less

Did you know?
A few years ago, Dutch health insurers commissioned an independent study into the cost price of a physiotherapy treatment.
After adjusting for inflation, the calculated cost price (without profit) came to approximately €54 per session.
Yet insurers ignored this outcome — and continue to reimburse physiotherapists only about €37 per session.
This forces many practices to work below cost price.
Some practices have reduced their treatment time from 30 minutes to 20 minutes just to stay financially afloat.
Why are we telling you?
Physiotherapy is an essential part of healthcare — representing only about 1.5% of total healthcare costs.
Due to increasing pressure, low fees and limited growth opportunities, more and more young therapists are leaving the profession.
That’s a loss — for the field, but especially for you as a client.

Do you need a supplementary package?

Would the most basic supplementary package be enough?

Note: If you expect to have surgery soon, a supplementary package may be beneficial.
Uninsured healthcare costs can sometimes be deducted from your taxable income in your annual tax return. You can find the conditions for this on the website of the Dutch Tax Authority.
Important government information
If you switch to another health insurer before 31 December, your old policy is automatically cancelled by your new insurer.

Build your own savings pot for additional healthcare instead of taking out a supplementary package. Depending on your situation, this can be more cost-effective — if not this year, then next year.

Are you self-employed? Check whether you can book your treatment costs (for yourself or your team) as business expenses.

Avoid the Big Four (VGZ, Achmea, CZ and Menzis). Their policies are becoming increasingly restrictive. The infographic at the top of this page shows which labels fall under these groups.
